Students’ Responsibilities:
• MUST pass the “Intro Exam” before you are allowed to leave to your job location!
• PHYSICAL COPIES of
- Picture ID (Driver’s License or Gov’t Issued ID) o Medical Insurance Card
- Vehicle Registration and Car Insurance WITH Student Listed (IF driving to and from school) OR Car Insurance Waiver (IF you are NOT driving yourself to work!)
- Current Paystub: MUST show taxes withheld and number of hours worked! Can NOT work for a relative! Must work a MINIMUM of 15 hrs/ week and start during school hours. Can NOT get paid via a cash app!
- Social Security Card (Need to physically see card or copy)
- Purchase of $14 School Insurance o Create an account
o Add Student + coverage on My Account Page o PRINT RECEIPT as proof of coverage o WWW.HSRI.com/K12_Enrollment/main/default.asp
- Fill out ONLINE and then Print = **DO NOT FILL OUT BY HAND!** o Cooperative Education Program Application – FORM 2409 o Training Agreement – FORM 4542 (MUST have EIN/Tax ID from company) o Media Release – FORM 7489 o Student Agreement – FORM 2413
- Must be a Junior or Senior with all graduation requirements covered and all EOC’s taken/passed with GPA of 2.0+ (No credit recovery)
When you have COMPLETED forms FM-2409, FM 2413, and FM-4542, and have everything signed, please come see Mr. Simmons to have your class schedule looked at!
